Mid-South Care Services in Middle Tennessee is seeking compassionate individuals and families to become Family Model Providers. You would host adults with disabilities in your home, offering a safe, supportive environment that encourages independence and dignity.

Responsibilities include maintaining a welcoming home, assisting with daily living, transportation, documentation, and following individualized support plans. Training and certifications are provided to help you succeed.

Qualifications

• Must be at least 18 years old.
• Safe, stable home with appropriate accommodations.
• Maintain homeowners or renters insurance.
• Maintain automobile insurance and reliable transportation.
• Complete all required Relias Training.
• Obtain and maintain Medication Administration Certification.
• Complete required background checks and screenings.
• Complete required home inspections and ongoing monitoring.
• Maintain all required certifications and compliance documentation.
• Demonstrate compassion, dependability, and strong communication skills.

Responsabilités

• Provide a safe, clean, and welcoming home environment.
• Assist with daily living activities, personal care, and individual needs.
• Promote independence, community involvement, and meaningful relationships.
• Follow each individual's PCSP.
• Provide transportation to appointments and community activities.
• Maintain accurate documentation and communicate with Program Coordinators.
• Ensure individual health, safety, rights, and regulatory compliance.
