Job summary

All Embracing Home Care is seeking compassionate Care Managers to provide in-home support and companionship, helping clients maintain independence and dignity in their own homes.

The role includes assisting with ADLs, medication reminders, light housekeeping, and coordinating with the care team. Travel between client locations is required; valid driver's license and reliable transportation are essential.

Responsibilities

• Maintain documentation and communicate with the care team.
• Create meaningful connections with clients and families through compassionate care.
• Travel between client homes; valid driver's license and reliable transportation required.
