Responsibilities

• Provides skilled speech therapy as ordered by the attending physician.
• Evaluates patient functioning using diagnostic and prognostic procedures.
• Develops an individualized ST plan of care and makes adjustments as needs change.
• Documents all aspects of treatment and patient education per agency protocol and Medicare guidelines.
• Promotes continuity of care with admissions, transfers and discharges.
• Instructs patient and family on disease, injury or disability management.
