Job summary

Baldwin Gardens Nursing Center in Temple City, CA is seeking a licensed Speech-Language Pathologist to evaluate and treat residents with communication and swallowing disorders to improve quality of life and functional abilities.

You will develop individualized treatment plans, provide articulation, language, cognitive-communication and swallowing therapy, monitor progress, document outcomes, and collaborate with physicians, nurses and therapists.

Qualifications

• Current license to practice as a Speech-Language Pathologist in the state of employment.
• Minimum of 2 years of healthcare experience, preferably in a skilled nursing.
• Knowledge of current speech therapy practice standards and regulations.

Responsibilities

• Evaluate resident's communication and swallowing abilities using standardized assessments
• Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders
• Provide skilled speech therapy interventions, including articulation and language therapy, cognitive-communication therapy, and swallowing therapy
• Monitor and document resident progress and adjust treatment plans as necessary
• Communicate with interdisciplinary team members, including physicians, nurses, occupational therapists, and physical therapists, to coordinate care and ensure optimal outcomes for residents
• Educate residents and family members on speech therapy goals, treatment plans, and home exercise programs
• Maintain accurate and timely documentation of speech therapy services provided
• Participate in team meetings, in-service education, and quality improvement initiatives
• Maintain current knowledge of speech therapy practice and relevant regulations
