Job summary

Liberty Health is seeking a Social Services Coordinator at Parkview Health and Rehabilitation Center in Chapel Hill, NC. The coordinator will conduct family conferences, document quality measures, and participate in required meetings while ensuring compliance with regulations and facility policies.

Qualifications

• Must read, know and follow personnel, department, and Facility policies.
• Must be knowledgeable of local, state and federal regulations as relates to social needs of patients.
• Must have one year of experience in health care or long-term care field or equivalent education/experience.

Responsibilities

• Plans and institutes new ideas and programs in harmony with the Facility.
• Interviews patients or family members for admission.
• Explains cost of care for appropriate level of care.
• Liaises between families, discharge planners, agencies and physicians.
• Notifies families of admission approval, room number and arrival time.
• Chairs the Admission Conference/MDS meeting for new admissions.
• Assists families with discharge placement and resources.
• Maintains list of community resources and assists in QA activities.
