Job summary

Racker is seeking a Self Directed Community Support Professional to deliver community-based services for individuals with developmental disabilities and their families. The role is co-managed with the service recipient and their family, aligning with individual service plans.

Qualifications

• High school diploma or GED.
• Must meet Approved Driver Status.

Responsibilities

• Ensure health and safety of individuals supported.
• Provide individualized services including respite, teaching, modeling, and skill development at home and in the community.
• Participate as a team member to advocate for the person and contribute to the Staff Action Plan.
• Follow objectives in the Staff Action Plan discussed with the person and team.
• Attend trainings to improve services and complete required documentation on time.
• Accurately enter time and services into the eVero health record and document service delivery and mileage as required.
• Use a smartphone for documentation and assigned tasks; personal device preferred.
• Contribute ideas and input regarding goals and services for the person receiving support.
• Maintain professional standards in behavior and dress; fulfill other duties as assigned.
