Qualifications

• Bachelor’s degree in human services or related field and prior working experience in mental health, OR a Licensed Practical Nurse with 2 years in mental illness/IDD settings.

Responsibilities

• Provides case administration to an assigned caseload.
• Facilitates group process and activity within the program.
• Contributes to the multidisciplinary treatment team process.
• Performs clinical assessments and develops/implements treatment plans.
• Communicates information regarding individuals to family and appropriate agencies.
• Collects data on client treatment plan goals.
• Shares in ongoing assessment of the individual’s progress.
• Engages in daily interaction and communication with program participants.

Education

Bachelor’s degree in human services or related field
