Job summary

Extendicare is seeking a part-time Registered Practical Nurse to deliver patient-oriented nursing care in line with provincial standards. You will work within an interprofessional team, manage assessments, plans and documentation, and support family education and transfers.

Responsibilities include direct resident care, medication administration, and coordinating care plans with the care team. Strong communication and clinical knowledge are essential, along with VPS screening within six months

Qualifications

• Current registration as a Registered Practical Nurse in applicable Province/State.
• Experience in a senior environment and with unregulated care providers is preferred.
• Education and experience in gerontology, rehabilitation, community/complex/long term care is a asset.
• Strong written and oral communication, critical thinking, care coordination, time management and customer focus skills.

Responsibilities

• Work as a member of the inter professional team applying Extendicare's Professional Practice Model with person-centered care.
• Complete and ensure resident's clinical assessments, care plans, documentation are accurate and updated.
• Implement and evaluate care plans and make changes as required.
• Assist with scheduling care conferences and participate when applicable.
• Collaborate with DOC/DHW for move in/out and transfers.
• Provide direct resident care and medication/treatment administration and document according to standards.
• Process orders and manage drugs/narcotics in compliance with regulations.
• Educate residents and families about care plans.
