Job summary

Responsibilities

  • Provide services to persons served in their home and community.
  • Perform lifting and other one-to-one functions for the individuals we serve.
  • Deliver direct care and skill-building services.
  • Monitor safety and welfare and notify supervisor of concerns.
  • Help individuals build positive relationships in their community.
  • Advocate for the needs and rights of people served; provide needed support.
