Ascend Health is hiring a Physical Therapy Assistant to oversee rehabilitation for residents in a home care setting. The PTA collaborates with an interdisciplinary team to balance outcomes with cost-effective care while adhering to regulatory standards.

The role requires an active PTA license, focus on older adult rehab, and ability to travel to other facilities as needed. This is a full-time, on-site position in a skilled home care environment.

Qualifications

• Current and Active Physical Therapy Assistant license within the state of operations
• Prefer experience/knowledge in older adult rehabilitation
• Prefer knowledge in governmental and regulatory guidelines in designated healthcare settings

Responsabilités

• Provide and direct resident rehabilitation care following accepted practice standards
• Evaluate resident needs and establish effective treatment plans
• Coordinate supervision of treatment plans with staff and other disciplines
• Screen residents according to directed schedules
• Provide feedback to the supervisor on clinical and rehabilitation issues
• Participate in center processes and services
• Manage treatment and related activities efficiently
• Complete clinical documentation to meet regulatory requirements
• Maintain billing records compliant with requirements
• Assist with marketing efforts as directed
• Travel to other facilities to provide services as needed
