Job summary

AccentCare is seeking a Physical Therapist (PRN) in Lombard, IL, with a focus on home health. The role requires evaluating and treating patients, coordinating with a multidisciplinary team, and providing patient education and care planning.

The ideal candidate will have a Master’s degree in physical therapy, a licensed physical therapist in the state of operation, and CPR certification. PRN hours are Monday–Friday, 8am–5pm, with travel to coverage areas as needed.

Qualifications

• Master’s degree in physical therapy approved by CAPTE-accredited program.
• Minimum nine months experience as a physical therapist.
• Currently licensed to practice as a physical therapist in the state of operation; CPR certification.
• Must be a licensed driver able to travel to all business locations.

Responsibilities

• Provides patient care and documents treatment per physician orders and agency policies.
• Performs patient evaluations per policy; selects appropriate diagnostic materials.
• Informs physician of assessment results and recommended clinical interventions.
• Develops and documents treatment plans with priorities and outcomes.
• Educates and counsels patients and families; maintains organized treatment documentation.
• Shares patient information with physicians and team members and updates on status.
• Meets productivity standards and manages time efficiently.
