Job summary

Oak Street Health seeks a Nurse Practitioner / Advanced Practice Provider to deliver high-quality primary care to adults on Medicare in underserved communities. You will conduct comprehensive assessments, manage chronic conditions, provide preventative care, and coordinate with physicians, nurses, and social workers.

You will educate patients and families, support care plans, and use data and EMR tools to improve outcomes.

Qualifications

• Active NP license in Texas is required or soon to be granted.
• Experience delivering primary care to adults is preferred.
• Familiarity with Medicare guidelines and patient education.

Responsibilities

• Provide primary care to adults on Medicare in underserved communities.
