Responsibilities

• Collaborate with providers, clinic nurses, practice managers, and other healthcare professionals to develop, implement, evaluate, and revise care coordination for high-risk patients.
• Coordinate delivery of high-quality healthcare services with providers and community agencies.
• Aim for prevention, assessment, risk reduction, and health status improvement.
• Connect with patients and families from diverse backgrounds to promote optimal health attainment.
• Work within a patient-centered, team-based approach with patients, families, and providers.
