You will collaborate with other health professionals and staff to maintain high-quality care.

Responsibilities include direct patient care, planning and documenting outcomes, and adhering to standards of care, safety, and infection control.

• Experience caring for SNF populations.
• Strong communication skills (verbal and written).
• Ability to travel as needed for job duties.
