Job summary

LCMC Health is seeking an Outpatient Oncology RN Navigator/OCN for our Louisiana facility. The role centers on advocating for patients, coordinating care across a multidisciplinary team, and serving as a clinical resource in specialty oncology care.

The navigator will educate, counsel, and support patients through the care continuum, from screening to survivorship, while partnering with service lines and community resources to optimize outcomes.

Qualifications

• Requires OCN certification and minimum 5 years oncology experience.
• RN licensure in Louisiana (LSBN) and BLS certification required.
• Bachelor's degree in Nursing (BSN) or eligible to obtain BSN within 3 years of hire.

Responsibilities

• Coordinate written and verbal communication between patient, family, physicians, and care team.
• Coordinate treatment plan across internal/external departments to ensure continuity of care.
• Assist patients in understanding diagnosis, options, and available resources.
• Serve as liaison within facility and community for oncology services.
• Provide education and information to patient and family to ensure seamless care.
