Job summary

Premier Healthcare LLC in Leroy Village Green is seeking a full-time Occupational Therapist (OT) to evaluate residents, develop rehab plans, and help patients regain independence in activities of daily living.

You will collaborate with the interdisciplinary team to provide resident-centered care, educate families, and ensure compliance with HIPAA and residents' rights while delivering compassionate, high-quality rehabilitation services.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Valid OT license and registration for the state of practice.
• Experience in long-term care or geriatrics is preferred but not required.
• Strong communication and organizational skills.
• Compassionate, team-oriented, and committed to resident-centered care.

Responsibilities

• Evaluate referred residents to determine functional level and rehabilitation potential.
• Provide direct care and develop individualized treatment plans to help residents regain independence in ADLs.
• Establish treatment goals based on residents' abilities, limitations, and physician recommendations.
• Implement therapeutic interventions to improve strength, coordination, mobility, cognition, and self-care skills.
• Educate residents and families on home programs and adaptive techniques.
• Complete documentation including evaluations, weekly progress notes, recertifications, and discharge summaries.
• Collaborate with the interdisciplinary team to ensure a resident-centered approach to care.
• Adhere to HIPAA and Residents' Rights and other regulations.
• Perform additional duties as assigned by the Director of Rehabilitation.
