Job summary

LiveWell Homecare Agency, LLC is seeking an Occupational Therapist to provide direct patient care and improve functional outcomes in a home health setting.

You will assess patients, develop personalized care plans, supervise treatments, and collaborate with a supportive clinical team to deliver compassionate, high-quality care. State licensure and CPR are required; a Master’s degree is preferred but a Bachelor’s with relevant experience will be considered.

Qualifications

• Current licensure or certification as per state regulations.
• CPR certification.
• Master’s Degree preferred; Bachelor’s Degree with relevant experience acceptable.

Responsibilities

• Providing direct occupational therapy care to patients.
