- Active Illinois Physician license must be obtained.

Responsibilities
- Facilitates entry of the client into the health care system.
- Provides coordinated, continuous, and culturally competent primary health care.
- Documents comprehensive client data according to organization standards.
- Assures access and continuity of care in designated hospital.
- Participates in quality improvement activities and peer review.
