Qualifications

• Master's or doctoral degree from CSWE-accredited school
• Active MSW license required
• One year of social work experience in a health care setting
• Current CPR certification
• Meets state and federal requirements

Responsibilities

• Assist patients and families with health-related financial, social and emotional concerns and coping skills.
• Provide social work services per the plan of care.
• Communicate changes in clinical status to the care team and office staff in a timely manner.
• Educate patients and families on social work interventions and treatment plan.
• Perform care planning, transfer and discharge functions.
• Identify risks in the patient's environment and implement interventions to reduce risks.
• Other duties as assigned.
