- Graduate of a vocational/technical nursing program.
- Current LVN/LPN license in the state of practice.
- Knowledge of lab procedures and EMR systems.
- Strong documentation and organizational skills.
Responsibilities
- Review medical records, identify gaps in care, and schedule preventive services per physician-approved protocols.
- Collaborate with providers, Nurse Navigators, and families to ensure continuity of care.
- Educate patients using evidence-based guidelines to promote engagement and health compliance.
- Maintain documentation accuracy, confidentiality, and compliance with all clinical standards.
