ViaQuest, Inc. in Monroe, OH is seeking a Developmental Disabilities Nurse - LPN for both part-time and full-time shifts (7am–7pm). The role pays $28–$29 per hour, with an additional +$4 per hour on weekends.

Qualifications

• Associate’s degree or equivalent in Nursing from an accredited nursing program.
• LPN license.
• Nursing experience preferably within Private Duty Nursing, Home Health or Disability Services is a plus but not required

Responsibilities

• Performing duties as outlined in the plan of care such as Medication Administration, Personal Care, Light Housekeeping and day to day support.
• 2:2 Ratio of individuals through a consistent caseload.
• Effectively communicating updates and changes to patient's status to the supervising RN.
• Maintaining a safe patient environment.
• Providing emotional and social support and compassionate care to patients.
• Maintaining current patient records and progress notes.
• Working as a member of the interdisciplinary team.
• Developing and establishing a plan of care with physician.
