Job summary

Humboldt Park Health in Chicago, IL is seeking a Licensed Social Worker - Care Management to join our on-site, full-time team. The role focuses on discharge planning, psychosocial assessments, and coordinating community resources to support patients and families.

You will participate in interdisciplinary rounds, develop person-centered care plans, and document activities in the medical chart. Requires MSW, LCSW licensure within 6 months, BLS, and 1–3 years of relevant experience; salary range

Qualifications

• 1-3 years of experience in similar role required
• 3+ years of experience in similar role preferred
• Master’s degree in Social Work required
• Licensed Clinical Social Worker licensure required within 6 months of employment
• BLS Required

Responsibilities

• Facilitates discharge planning from admission and assesses patient needs to prevent delays
• Active participation in IPOC rounds with Case Manager, Nurses, and Physicians
• Prepares psychosocial evaluations and care plans age appropriately
• Assesses psychosocial needs of patients, caregivers, and families and documents
• Develops discharge plans and makes timely referrals to community services
• Implements plan of care with focus on client self-determination
• Uses individual, couples, and group modalities to address psychosocial needs
• Maintains up-to-date records of social work activities in the chart
