Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Portsmouth, OH. The role is PRN, focusing on delivering high-quality speech-language pathology services on-site with CMS-compliant documentation in NetHealth EMR.

Responsibilities include evaluation, treatment planning, family and staff instruction, FEES/MBS protocol oversight, and participation in resident needs screening to ensure timely care and clinical excellence.

Qualificações

• Master's degree in Speech-Language Pathology is required.
• Certificate of Clinical Competency (CCC) is required.
• Active SLP license in state(s) of practice is required.
• Commitment to ongoing continuing education to maintain licensure and certification.

Responsabilidades

• Provide quality speech therapy services in an in-house Skilled Nursing Facility.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and goals (short-term and long-term).
• Instruct patients, families, and staff to ensure carryover of therapy in daily activities.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to identify resident needs and meet them timely.
