Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Home Health Clinical Manager in Gadsden, AL. Lead a clinical team delivering high-quality, 1:1 patient care in the home setting and oversee care plans, compliance, and performance metrics.

You will collaborate with leadership to drive quality initiatives, supervise staff, and ensure standards across patient care while maintaining autonomy within a supportive team environment.

Qualifications

• Bachelor’s Degree preferred in Nursing from an accredited school.
• Licensed RN in good standing in the state of practice.
• Minimum two years of clinical experience, and at least one year in a related management/supervisory role in home health care.
• Current CPR certification.
• Ability to communicate (verbal and written) with all levels of personnel, internal and external to the company.
• Ability to work independently as well as part of a team.

Responsibilities

• Conduct quarterly quality improvement meetings, review data, identify trends, and oversee improvement plans.
• Collaborates with the Branch Director in implementing quality improvement activities and corrective action plans.
• Oversee external and internal surveys and audits within the assigned area relating to quality of care and KPIs with operations leadership.
• Complete documentation and chart reviews to identify variances and opportunities for improved patient care and organizational performance.
• Holds case conferences to ensure oversight of care and coordination of services.
• Facilitate development of performance improvement action plans based on data and survey results.
• Ensure assigned staff are evaluated fairly and timely according to company procedures.
