Qualifications

• Education: Associate’s Degree or diploma in nursing.
• Experience: One year nursing experience or one year nursing experience in a specialty.
• Licensure: Registered nurse with a license to practice in CT.
• Preferred: NLN approved BSN program.

Responsabilités

• Provide skilled nursing care to clients in home and community-based settings.
• Maintain client records using the Agency’s documentation standards.
• Conduct follow-up visits to assess disease progress and response to treatment.
• Educate patients and caregiver on health maintenance and disease prevention.
• Coordinate care with physicians and other providers.
