Job summary

CARE FIRST HOME HEALTH CARE INC in California is seeking a Physical Therapist for home health services. The therapist provides therapy under physician plan of care, assesses patient needs, and coordinates with family and caregivers.

Responsibilities include directing therapy, training families, documenting progress, and participating in discharge planning. Requires CA PT license, 2 years PT experience, CPR, and valid driver’s license; favorable home health experience.

Qualifications

• Bachelor's or master's degree in physical therapy from an accredited program.
• Licensed to practice as a physical therapist in California.
• Two years of physical therapy experience; home health experience preferred.
• Strong verbal and written communication and organizational skills.
• Current CPR certification.
• Valid driver's license and reliable vehicle.

Responsibilities

• Provides physical therapy services to patients according to a physician's plan of care and the state Physical Therapy Practice Act.
• Assessing and evaluating therapeutic/rehabilitative/functional status and contribute to the plan of care.
• Assess muscle strength, mobility, gait, ROM for rehab potential.
• Directs physical therapy treatment.
• Instructs patients and families in use and care of therapeutic appliances.
• Trains patient in prosthetic device use when applicable.
• Prepares and submits progress summaries and participates in discharge planning.
• Supervises PT assistants and home health aides as required.
