Zusammenfassung

River Valley Home Health and Hospice is seeking a licensed Physical Therapist to provide skilled services in the patient’s home under a physician's plan of care. You will evaluate functional status, mobility, and home environment to develop and implement a comprehensive plan of care.

You will supervise therapy assistants, educate patients and families, and coordinate with other home health services. A valid TX license and CPR are required, with at least two years of experience.

Qualifikationen

• Requires a degree from a baccalaureate or Master’s program in physical therapy approved by an accredited organization.
• Licensed to practice as a physical therapist within the state, if applicable.
• Two years of appropriate experience as a physical therapist; home health experience preferred.
• Demonstrates good verbal and written communication and organization skills.
• Maintains CPR certification and valid driver’s license with a reliable vehicle.

Aufgaben

• Provide physical therapy services to patients per physician's plan of care and state Practice Act.
• Assess and develop the total plan of care, evaluating functional status and home environment.
• Evaluate muscle strength, mobility, gait, ROM and rehab potential.
• Direct physical therapy treatment and instruct patients and families in use of therapeutic appliances.
• Prioritize therapy needs, report patient responses to treatment to the physician.
• Initiate therapy programs and train other personnel and family caregivers as needed.
• Instruct patients in prosthetic device use and assess needs for other home health services.
• Prepare and submit progress summaries according to policy; provide inservice education as needed.
• Supervise physical therapy assistants per policy and state regulations; supervise home health aide care when therapy is sole skilled service.
