Job summary

Compassionate Care Home Health in Bakersfield, CA seeks a licensed Physical Therapist to provide therapy services at patients’ homes, based on physician plans and state regulations.

Location includes Bakersfield and surrounding areas. Compensation is hourly with Paid Per Visit (PPV) under a hybrid model; negotiation based on experience and availability. Join a team focused on compassionate, clinical excellence and growth.

Qualifications

• Must hold a Master’s or Doctoral degree in Physical Therapy.
• Licensed to practice as a physical therapist in the state.
• Minimum of 1 year of experience as a physical therapist; home health experience preferred.
• Strong verbal and written communication; well organized.
• Current CPR certification and valid driver’s license.

Responsibilities

• Provide physical therapy services per physician's plan of care and state PT Act.
• Assess rehab status and plan care; evaluate home environment.
• Direct PT treatment and instruct patients and families on use of therapeutic devices.
• Identify needs for other home health services and refer as necessary.
• Prepare and submit progress summaries; participate in discharge planning.
• Supervise PT assistants and train home health aides as needed.
