Job summary

DFW Home Health, part of the LHC Group and within the Optum family, is hiring an Occupational Therapist to assess and treat patients in home settings. You will evaluate functional status, activities of daily living, and home safety, coordinating with the care team to develop and adjust plans of care.

You will provide documentation, participate in ongoing communication with physicians, and support patient mobility and independence in diverse home environments.

Qualifications

• Current Occupational Therapy licensure in the state of practice.
• Current CPR certification.
• Strong communication and organizational skills.
• Ability to work in diverse home-care settings.

Responsibilities

• Assess and evaluate patient care needs related to functional status and daily living activities.
• Collaborate with physicians and care team to develop therapy plans.
• Document patient progress and report changes to nursing staff and physicians.
• Conduct home assessments and recommend adaptive equipment as needed.
