Job summary

SunCrest Home Health, part of the LHC Group within the Optum family, is seeking an Occupational Therapist Assistant for per diem home visits. The role provides occupational therapy in patients' homes, helping restore maximum physical function under supervision.

Responsibilities include following the plan of care, documenting progress, and educating patients, families, and care teams about therapy and safety in diverse home environments.

Qualifications

• OTA licensure in the state of practice.
• Driver's license with vehicle or access to transportation.
• CPR certification preferred, or able to obtain within 90 days.

Responsibilities

• Provide occupational therapy to home care patients.
• Follow plan of care and supervise OT guidance.
• Document patient progress and communicate with nurses and physicians.
• Instruct patients and families on therapy and safety in home setting.
