Working Nurses Homecare seeks an in-person Clinical Case Manager in Margate, FL. You will assess client needs, develop treatment plans, and coordinate services with a multidisciplinary team to improve health outcomes at home.

Qualifications

RN or LPN license required.
Experience in home health care settings.
Strong knowledge of Medicare rules and regulations.

Responsibilities

Assess and plan client care; develop and monitor treatment plans with interdisciplinary team.
Admit new clients; complete intake interviews and orientations.
Coordinate services; arrange transportation and other resources.
Monitor client progress and document outcomes.
Maintain accurate client records and progress notes.
Conduct weekly interdisciplinary meetings to review progress and adjust plans.
Assist in discharge planning and family training for post-discharge care.
Support staff through education, training, and process improvements.
Monitor budgets and cost-saving opportunities.
Prepare reports on treatment results and trends.
