Qualifications

• Doctor of Medicine degree or Doctor of Osteopathy degree from an accredited medical educational institution.
• Successful completion of an accredited residency program.
• Board Eligible or Board Certified in their clinical specialty.
• Active Arizona state medical license including prescriptive authority.
• Must obtain and maintain a DEA license for prescribing controlled substances.
• Must obtain and maintain Basic Life Support (BLS) certification.
• Level I fingerprint clearance card and reliable transportation with valid driver's license and insurance.

Responsibilities

• Provides high-quality medical care to new and established patients.
• Prescribes medications and treatment regimens.
• Maintains comprehensive medical records and adheres to regulatory standards.
• Participates in continuing medical education and committee activities.
