Job summary

PacificSource is seeking a qualified Clinician Care Manager to support members in navigating health and human services. The role emphasizes advocacy, coordination of care, and resource management to promote optimal health outcomes.

The position requires an active RN license or equivalent clinical credential (LPC/LMFT/LCSW) and CCM certification either at hire or shortly after, with collaborative work across care teams to ensure high-quality patient care.

Qualifications

• Active, unrestricted RN license is required.
• LPC, LMFT, or LCSW credential required.
• CCM certification required at time of hire or within two years.

Responsibilities

• Facilitate client wellness and autonomy through advocacy, assessment, planning, communication, education, resource management and service facilitation.
• Develop comprehensive, individualized care management plans based on client history and needs.
• Link clients with appropriate providers and resources across health and human services.
• Maintain client privacy, confidentiality, health and safety in line with ethical and regulatory standards.
• Collaborate with PacificSource personnel and answer medical or contract interpretation questions.
