Job summary

Tucson Residence Foundation seeks a Residential Float Mentor to provide direct support and supervision for individuals served in our programs. You will assist with daily living activities, skill development, nutrition planning, and transportation needs in a home-style setting.

Role requires driving license, strong communication, computer skills, and the ability to pass background checks. On-call hours and flexible scheduling are part of the position, with emphasis on safety and client

Qualifications

  • Requires current valid Arizona Driver's License.
  • Strong verbal and written communication skills.
  • Proficient computer use.

Responsibilities

  • Provide active direct physical assistance, supervision and behavioral support to individuals in Agency programs.
  • Demonstrate safe workplace behavior per safety plan.
  • Provide opportunities for individuals to acquire and maintain skills for independence.
  • Maintain ongoing knowledge of individuals served and respect privacy/confidentiality.
  • Serve as a positive role model and assist in developing written objectives and skill plans.
  • Carry out skill development plans and routine tasks as required by the Agency.
  • Assist individuals served in planning social, recreational, and educational activities.
  • Support problem solving and decision making for individuals served.
  • Assist with planning, purchasing and preparation of food within budget and support nutritional intake.
  • Assist with managing funds for individuals served.
  • Assist with personal hygiene and health & safety issues.
  • Obtain, administer and maintain medications; provide transportation.
  • Attend and document meetings and medical appointments; provide feedback.
  • Purchase household and hygiene supplies as required.
  • Maintain housekeeping and home maintenance to ensure a neat, safe environment.
  • Conduct evacuation and emergency drills; maintain records.
  • Maintain Agency equipment and follow maintenance guidelines.
  • Work on an on-call schedule, up to five days/40 hours per week; respond within 15 minutes.

Job description

Position: Residential Float Mentor

Reports to: Assistant Director of In-Home Services and On-Call

Supervises: No positions

Primary Hours: Set based on needs of individuals in service

40 hours per week

Required

Qualifications: Current valid Arizona Driver's License

Able to communicate effectively with individuals in service and team members using both verbal and written skills

Physical Requirements:

Must be able to lift 50 pounds to help provide support and safety for individuals supported by lifting, moving, positioning, and transporting individuals served; May be required to use Hoyer Lifts, Wheelchairs, gait belt, etc.)

General Responsibilities:

  1. Provide active direct physical assistance, supervision and behavioral support to individuals served in Agency programs
  2. Demonstrate safe workplace behavior as defined by Agency workplace safety plan

Specific Duties and Responsibilities: Individuals Served

  1. Provide opportunities for members to acquire, use and maintain skills which enable individuals served to live as independently as possible
  2. Maintain an on-going knowledge of the individuals served
  3. Maintain the privacy and confidentiality of individuals served
  4. Serve as a positive role model for individuals served
  5. Assist in the development of written objectives and skill plans
  6. Carry out skill development plans, recommendations, agreements, assignments, and routines as required by the Agency
  7. Assist individuals served in planning and participating in social, recreational, and educational activities at home and in the community
  8. Actively assist individuals served with problem solving
  9. Assist individuals served with planning, purchasing and/or preparation of food to ensure nutritional intake within the context of the budget
  10. Assist with and/or manage funds for individuals served as assigned
  11. Assist individuals served with personal hygiene, and health & safety issues that affect them.
  12. Obtain, administer, and maintain medications for individuals served
  13. Provide transportation for individuals served
  14. Attend, document and provide feedback for meetings and/or medical appointments for individuals served
  15. Purchase household and hygiene supplies as required
  1. Operations/Equipment
  1. Provide routine and on-going housekeeping and home maintenance of interior and exterior of the residence to provide a neat, clean, healthy, and safe environment
  2. Conduct and participate in evacuation and emergency plan drills and document the results
  3. Maintain and utilize all Agency equipment and appliances in a manner which will promote their longevity (i.e. observe maintenance schedules, repair as needed, follow manufacturer's directions for use, etc.)

Professional/Operational

  1. Work an agreed-upon schedule in an on-call capacity, up to five days per work or 40 hours
  2. Maintain active availability during assigned times, responding within 15 minutes to program-related communications and providing prompt support for staffing coverage, operational issues, and emergency situations.
  3. Requests and notifies Assistant Director of leave times in accordance with Agency procedure
  4. Interact and communicate with individuals served, their team members, other Agency personnel, and members of the community in an appropriate and professional manner
  5. Document the progress/status of individuals served and maintains files and records in accordance with Agency standards using Agency record keeping systems
  6. Attend required Agency and Agency-related meetings as directed
  7. Comply with all Agency Policies and Procedures as well as all applicable regulations and laws
  8. Acquire and maintain all certifications and training required by the Agency in a timely manner
