Job summary

Holon Health in Illinois is seeking a mission-aligned Community Relationship Manager to grow our presence across the state. You will identify and cultivate partnerships with community organizations, healthcare providers, health plans, and justice-system partners, helping these stakeholders understand how Holon’s care model can change lives.

You'll mentor the Client Acquisition team, deliver training for justice and community partners, and drive regional growth.

Qualifications

• Bachelor’s degree or equivalent professional experience.
• Experience in account management, community engagement, outreach, or sales in healthcare/behavioral health/criminal justice.
• Experience leading teams and driving results.
• Proficiency with CRM tools and virtual collaboration.
• Travel across Illinois required.
• Experience with community orgs, healthcare providers, health plans, or justice partners is a plus.

Responsibilities

• Build relationships with referral partners across multiple channels.
• Serve as primary contact for partners in your territory.
• Conduct in-person visits, presentations, and trainings.
• Represent Holon at local/state conferences and events.
• Identify opportunities to co-host educational initiatives.
• Act as a visible ambassador for Holon’s mission and model of care.
• Track industry trends and inform outreach strategy.
• Foster an inclusive, equity-driven team culture through training.
• Other duties as assigned.
