Zusammenfassung

1st Choice, LLC is seeking a CAS Assessor for IDD services in New York to support statewide initiatives. You will conduct person-centered assessments using the CAS tool to guide individualized care and service planning, traveling within your assigned region and providing in-home and community-based assessments.

Ideal candidates will hold a bachelor’s in human services (master’s preferred) and have at least one year of direct IDD experience.

Qualifikationen

• Bachelor’s degree in a human services field is required.
• Master’s degree in a related field is preferred.
• Experience with CAS or similar assessment tools is advantageous.

Aufgaben

• Conduct in-person CAS assessments in homes and community-based settings.
• Travel throughout the assigned region to meet with individuals, families, and caregivers.
• Use a person-centered approach to assess needs, strengths, and support requirements.
• Interview caregivers, service providers, and other stakeholders to gather accurate information.
• Review relevant records and incorporate multiple data sources to support assessment coding.
• Enter data into state-mandated systems and ensure timely, accurate documentation.
• Maintain productivity and quality standards as outlined by the program.
• Coordinate interpreter services as needed for non-English/Spanish-speaking participants.
