Job summary

Responsibilities include conducting comprehensive patient assessments, formulating and documenting individualized care plans, and collaborating with the interdisciplinary team to meet patient needs. A NY RN license and BLS certification are required; a BSN is preferred.

Qualifications

• Diploma or Associate's Degree in Nursing.
• Registered Nurse license in New York State.
• Basic Life Support (BLS) certification.

Responsibilities

• Provides high-quality, patient-centered clinical care.
• Performs initial and ongoing assessment of the patient, utilizing data from physical, psychological, educational and risk assessments.
• Communicates and documents assessment with the interdisciplinary team and utilizes resources to meet patient and family needs.
• Develops and documents an individualized plan of care and services.
• Complies with policies, procedures, and safety, medication and documentation protocols.
• Revises plan of care based on ongoing evaluations and updates documentation accordingly.
• Completes at least one CRU form for Nursing Research submission with a relevant US-based article.
• Completes a minimum of 5 nationally approved contact hours (CNE or CEU) per year.
