Qualifications

Responsibilities

• Collects data pertinent to patient health, analyzes data, and develops an intervention plan.
• Collaborates with patient, family and healthcare team to provide safe, compassionate care and education.
• Participates in discharge planning to ensure continuity of care.
• Delegates tasks and coordinates duties of the healthcare team.
• Performs other related job duties as assigned.
