Responsibilities

Performs procedures, monitoring, or other functions as ordered by the medical provider(s). Documents the administration of care in the patient medical record in a timely and thorough manner.
Teaches patients/families/caregivers about medical conditions, status, treatment plans, medications, side effects and follow-up measures.
Explains non-medicinal follow-up measures, such as diet and exercise, to enable self-management.
Manages evidence-based practices and continuous process improvements to meet quality measures.
Collaborates with a multidisciplinary team to coordinate and deliver a holistic patient treatment plan.
