Mercy Hospital Joplin is seeking a Clinical Pharmacy Specialist - ICU to optimize medication therapy for critically ill patients and collaborate with physicians, nurses, residents, and the health care team.

Qualifications

• BS or PharmD required.
• Current Pharmacist License in the state of practice.
• Completion of a PGY-1 Residency or 4 years of hospital pharmacy experience in a specialty area.
• Missouri pharmacists must obtain a Medication Therapy Services Certificate within 90 days of hire.
• Preferred PGY-2 Residency in Critical Care or related specialty.
• Four or more years of clinical pharmacy experience.
• Board of Pharmacy Specialties (BPS) Certification or eligibility.
• Previous experience precepting pharmacy students or residents.

Responsibilities

• Review and optimize medication therapy for critically ill patients.
• Collaborate with providers to develop individualized treatment plans.
• Monitor patient outcomes and adjust therapies to ensure safe and effective care.
• Identify and resolve medication-related issues, adverse reactions, and drug interactions.
• Provide drug information and clinical consultation to physicians, nurses, and patients.
• Participate in interdisciplinary rounds and patient care discussions.
• Support formulary management, policy development, and quality initiatives.
• Document clinical interventions and medication therapy recommendations.
• Serve as a clinical resource and educator for pharmacy staff, students, and residents.
• Contribute to medication safety and regulatory compliance efforts.
