Hamilton Health Box is a growing healthcare startup delivering hybrid telemedicine-enabled primary care clinics for small and medium businesses. The Advanced Practice Provider collaborates with supervising physicians to manage 3-5 micro clinics, delivering health coaching, disease management, preventive screening, and education through a mix of virtual and in-person care.

The role requires clinical expertise, strong communication, and teamwork to improve patient outcomes and program efficiency

Formación

• Graduate of a Nurse Practitioner program approved by the State of New Mexico; Master of Nursing required.
• Licensure by the State of New Mexico APRN and eligibility for full prescriptive authority.
• DEA licensure eligibility.”

Responsabilidades

• Provides medical services under physician supervision to promote patient health.
• Identifies short- and long-term patient care issues to improve physician efficiency.
• Orders or interprets diagnostic tests and documents results.
• Delivers injections, immunizations, wound care, and other procedures.
• Develops patient-management plans and educates patients on health maintenance.
