Qualifications

• Licensed physical therapist with CA license eligibility or active CA PT license.
• Completion of an accredited PT training program (Bachelor/Master/Doctorate).
• Strong clinical reasoning and documentation skills.

Responsibilities

• Evaluate patients and determine dysfunction and goals.
• Establish plan of care and goals for each patient.
• Prescribe or administer modalities and exercise programs.
• Document services in accordance with policies and regulations.
• Coordinate discharge planning with health care team.
• Supervise PT assistants and aides as appropriate.
• Provide patient and family education.
