• 1+ year of experience in Geriatric care and Documentation review.
• Excellent communication, collaboration, and teamwork skills.

Responsibilities

• Provide high-quality preventative care to residents in skilled nursing facilities.
• Conduct on-site visits and remote work from home, supported by a dedicated scribe team.
• Build strong relationships with facility staff and residents.
• Maintain a patient-centered approach and provide collaborative care.
• Utilize remote patient monitoring, chronic care management, and collaborative care management programs to reduce hospitalizations and improve patient outcomes.
