Qualifications

• Minimum one year of experience as a Medical Assistant; Family/Internal/Specialty Medicine preferred.
• Minimum one year of referral coordination experience.
• Knowledge of medical terminology, online scheduling, universal precautions, and body mechanics.

Responsibilities

• Assist NP/MD/RN with scheduling, coordinating and communicating care.
• Engage patients via high-volume telephonic outreach to enroll in Population Health programs.
• Support the Emergency Department to Home Program with orders and provider communication.
• Gather intake information and escalate issues to an RN as needed; ensure accurate patient records.
