Financial Planning Plus in Charleston, SC is seeking a Licensed Practical Nurse to join our hybrid schedule healthcare team. The role focuses on precision and compassionate care, with responsibilities across geriatric labs, shift handoffs, and coordination with care teams.

You'll work across Charleston sites, support telemonitoring workflows, and guide patients and families through decisions with clear communication.

Formación

• Experience translating complex decisions for non-technical audiences.
• Ability translating jargon into plain English.

Responsabilidades

• Prep and verify Geriatric Care specimens for the lab, labeling at the bedside in front of the patient
• Hand off at shift change using SBAR, leaving zero gaps for the incoming Licensed Practical Nurse
• Hold chemotherapy administration follow-up calls within 48 hours of discharge
• Communicate clearly with patients, families, and care teams
• Manage caseloads efficiently in a fast-paced Charleston clinical environment
• Map each patient's pain across the shift and adjust comfort measures before they ask twice
• Bridge the language gap with interpreter services so consent is truly informed in Charleston
• Cover float assignments across Financial Planning Plus's Charleston sites without losing a beat on continuity
