Job summary

Community Counseling-Bristol is seeking a Supportive Housing Case Manager to enhance services for unsheltered individuals in Bristol County. The role involves providing intensive case management, coordinating services, and fostering relationships with community members.

The ideal candidate will have a Bachelor's degree in Social Work or related field, with experience working with vulnerable populations. The position offers a hybrid work option and benefits including health insurance and paid vacation.

Qualifications

• Bachelor's degree in Social Work, Sociology, Psychology, or Human Services related field OR 3-5 years of experience working with homeless populations.
• Knowledge of available referral sources for providing services to clients.
• Ability to drive throughout the county and at times transport clients required.

Responsibilities

• Coordinate with client, medical, and mainstream providers for wrap around services.
• Develop programming to help clients remain housed.
• Assist clients with applications for subsidized housing.
