Qualifications

• Social worker licensure required in Wisconsin.

Responsibilities

• Perform psychosocial screening and assessment to identify needs of outpatients and families.
• Develop goals aligned with patient health goals and coordinate resources.
• Provide psychosocial, counseling, and case management services.
• Educate patients and families on Advance Care Planning and assist with documentation.
• Facilitate transitions to home and community and collaborate with care providers.
• Support patient navigation and advocacy to improve care access.
• Participate in research activities and adhere to ethics and standards.
