Equiliem seeks a CAS Assessor to arrange, conduct, and complete accurate assessments using a person-centered approach for OPWDD services in New York. Assessors coordinate with individuals, families, caregivers, and care managers across home, community, or telehealth settings.

The role emphasizes scheduling, conducting interviews, documenting activities, ensuring quality and productivity, arranging interpreter services when needed, and using the CAS software to finalize assessments.

Qualifications

• Bachelor's degree in an approved human service discipline.
• Minimum of one year experience working with individuals with intellectual and/or developmental disabilities.
• Ability to conduct person-centered interviews and coordinate assessments with families and providers.

Responsibilities

• Outreach to individuals and caregiver/primary supports during the schedule.
• Schedule and coordinate assessments in home, community, or via telehealth.
• Conduct assessments with a person-centered interview style.
• Coordinate interpreter services as needed and document all case activity.
• Meet productivity and quality standards and participate in training.
