Qualifications

• Needs assessment to evaluate client situations and set realistic objectives.
• Case management to develop and oversee service plans.
• Identify barriers and coordinate services to support employment goals.

Responsibilities

• Develops individualized service strategies based on needs assessment and assist customers in attaining goals.
• Conducts interviews to articulate program services and identify components applicable to customer need.
• Manages caseload, provides case management and referrals; removes barriers and documents needs.
• Determines eligibility and explains program participation requirements; develops employment contracts.
• Develops individual employment plans and guides customers through programs and services.
• Performs job development, matches, follow-up, referrals, and verifies job search.
• Provides outreach and follow-up services and coaching to address employment barriers.
