Optum Behavioral Care in Arizona seeks a Behavioral Health Clinician to join the team embedded in primary care practices across Glendale, Goodyear and Surprise. You will deliver brief, evidence-based behavioral health care to adults, including the geriatric population, within a collaborative care framework.

You will work with physicians and care teams, combining brief psychotherapy, care planning, and symptom monitoring to improve outcomes.

Qualifikationen

• Active, unrestricted behavioral health license in Arizona (LMSW).
• 1+ years of experience providing behavioral health treatment.
• Experience assessing and treating common mental health conditions.
• Proven comfort delivering brief, evidence-based interventions.
• Proven interest in working in a multidisciplinary, team-based care model.
• Proven comfort working with diverse patient populations across mental health and medical conditions.

Aufgaben

• Provide brief, evidence-based behavioral health interventions using in-person and virtual modalities.
• Screen, assess, and diagnose common mental health and substance use disorders.
• Collaborate closely with primary care providers, psychiatric consultants, and care team members.
• Track patient progress using measurement-based care and adjust treatment as needed.
• Support psychotropic medication management by monitoring adherence, side effects, and treatment response.
• Participate in regular psychiatric case consultations and communicate recommendations to providers.
• Develop relapse-prevention and self-management plans for patients ready for discharge from active care.
• Maintain accurate documentation and follow collaborative care workflows.
